Details of complaints received and any reported illnesses:

• Glass complaint received Oct 4, 2024. Investigation from packer determined incident was isolated. So addressed the current complaint with customer. However on Oct 23, complaint of another possible glass was received, thus complaint was escalated to withdrawal of product. All inventory has been placed on hold.
• There has been no report of illness.

Details of investigation and assessment:

• Glass complaint received Oct 4, 2024. Investigation from packer determined incident was isolated. So addressed the current complaint with customer. However on Oct 23, complaint of another possible glass was received, thus complaint was escalated to withdrawal of product. All inventory has been placed on hold.
• There has been no report of illness.

Details of decision regarding the affected products:

• 1100 cases (6 x 1.6L) had been imported from Wadi El Nile (Egypt)
• 601 cases on inventory has been placed on hold pending completion of all investigation. Plan to destroy once all investigation & returns are completed.
• Issued withdrawal letters to 2 customers who have received the products (attached) – 240 cases & 258 cases, inventory variance of 1case – all returns/product traced in the supply chain to be destroyed
