Details of complaints received and any reported illnesses:
- First complaints of glass in product were received Oct 4, 2024.
- Additional complaints received October 23, 2024
- There has been no report of illness.
Details of investigation and assessment:
- Initial glass complaint investigation from packer determined incident was isolated and addressed the complaint with customer.
- Following the addtional complaints, the complaint was escalated to withdrawal of Wade El Nile olive products. All inventory was placed on hold and eventually destroyed.
Details of decision regarding the affected products:
- 1100 cases (6 x 1.6L) had been imported from Wadi El Nile (Egypt)
- 601 cases on inventory has been placed on hold pending completion of all investigation. Plan to destroy once all investigation & returns are completed.
- Issued withdrawal letters to 2 customers who have received the products (attached) – 240 cases & 258 cases, inventory variance of 1case – all Wade El Nile olive returns/product traced in the supply chain to be destroyed
